FIRST VISIT

 

We take the time to get to know you.

For your first visit with Integrative Medical Associates please set aside 90 minutes to 2 hours. During this time, we listen and discuss with you at length about your medical history, lifestyle and health concerns. We perform a general exam and determine if you need any diagnostic tests based on your history and examination.

The goal of our first meeting is to gather information about all aspects of your life. Based on our conversations and your physical exam, we'll make recommendations that are compatible with you, your needs and beliefs. For some patients, the recommendations may include nutraceutical/botanicals therapies. For others it maybe a lifestyle change, diagnostic testing or conventional medical therapeutics.

Before Your Visit

We have our new patient forms online. You can print them and complete the forms at your convenience. When you come to see us please bring the following items:

  • New Patient Forms
  • Insurance Card
  • List of medications and supplements you are currently taking
  • A copy of any labs or other diagnostic tests you have had done in the past 6 months.

Insurance

While integrative medicine is growing in popularity among healthcare consumers, many insurance companies do not provide coverage for these services. At this time due to insurance limitations, the consultation will remain on a fee for service basis. We do however offer a courtesy insurance billing, provided we have current and accurate insurance information. Depending on your insurance guidelines, we will submit your visit to your insurance company for possible reimbursement. Medicare does not cover integrative medicine.

Fees

As most insurances do not currently provide coverage for integrative medical services, all fees are due at the time of service. We accept cash, check, Visa and Mastercard.

Cancellation Policy

We have exclusively reserved the doctor, nurse, or staff and facilities for your personal health care.  We ask our patients to give us a 24 hour notice if you need to cancel or reschedule.  If you do not come in for your appointment, or break you appointment without sufficient notice, a $50.00 per hour broken appointment fee will be applied to your account and you will be required to pre-pay for your next appointment.  For IVs and injections, the fee will be the cost of the IV or injection, unless there is an available time slot for the patient to be rescheduled within 48 hours.  If you are scheduled for multiple appointments for the same day, and you do not come in for your appointment, or break you appointment without sufficient notice, a $50.00 per hour broken appointment fee will be applied to your account for each appointment missed and you will be required to pre-pay for your next appointment.
