Male Menopause

One of the most common concerns I hear from both male and female patients is, "Doctor, I just don't have the stamina and strength I used to have."

This month we will address the issue of male menopause, a.k.a. andropause. As men age, they have a decline in testosterone and DHEA, and some males may experience an increase in estrogen, consequently more adipose tissue is deposited in the abdomen and chest. It is important that the male have both the anterior pituitary and testicles in communication to maintain proper hormone balance.

The typical male may first notice this change in hormones starting in his mid to late 30's and will show a significant difference in his 50's. Symptoms include increased muscle pain with exertion, decreased exercise tolerance, decreased grip strength, increased depression, lack of motivation, decreased libido/sex drive, decreased quality of erections, and decreased gonad size. It is important to note that this change is gradual. The patient will often observe that after a major stressor (i.e.,such as a divorce, death of a loved one, loss of a job, etc.) his vigor and vitality are not what they used to be.

Several laboratory tests can be used to detect andropause. For example, Testosterone (Total and Free), PSA, CBC, chemistry and thyroid panel. It is critical to obtain laboratory test results to help with determining the proper therapies. Proper blood levels of testosterone can be beneficial for enhanced libido, improved cardiac function, blood sugar regulation, balanced metabolism and stronger immune function.

Prescription testosterone is often helpful along with natural replacement therapy which can enhance the drug therapy. Some botanical that help are Gingko Biloba to increase blood flow to the penis and Ginseng to increase energy and erections. For Erectile Dysfunction, Muira puama, Daminana and MACA are effective. To increase libido, DHEA can be beneficial; however, do NOT take if you have prostrate problems. For prostrate, consider taking the following daily: zinc chelate 30 - 60 mgs, Saw Palmetto 320 mgs, copper 1 - 3 mgs, and pygeum 100 mgs.

As far as diet is concerned, decrease sugars and fried foods because these cause blood vessels to become clogged and can lead to impotence. Utilize a good grade protein powder and use plenty of flax seed oil and olive oil with foods.

Lifestyle changes include mild aerobic exercise along with light weight training as this will help maintain proper muscle tone. Increase sleep and decrease stress with medication, tai chi and yoga. Avoid small bicycle seats as they can cause problems with nerves to the penis and prostate.

In closing, with proper hormonal regulation, they symptoms of andropause can improve and put you on the path to vitality.
